romance animes with actual good character designs and pure romance?
only one i could find is fragrant flowers and the angel next door and i literally rewatched it already after like 3 months since i couldnt find any others

Teasing Master Takagi-san is as sweet and wholesome as The Angel Next Door, but with a lot more comedy in it and more episodic (there's a bit of continuity between episodes and overarching progress in their relationship, but there's not a continuous plot like in Angel Next Door).
Love, Chunibyo and other delusions has also more comedy than Angel Next Door, but has more of a plot than Takagi-san, and the characters are just great.
If you liked Mahiru as a character, you might like Aoi, from Ai Yori Aoshi, it's a harem, but different than other harems becayse the main couple gets together almost from the start, but they have to keep their relationship a secret, thus allowing other girls to develop feelings for the MC, but the MC doesn't have feelings for anyone but the main girl.. The problem is that the second season doesn't end in a very satisfying way.
Also, if you don't mind an 80's romcom about a love triangle, Kimagure Orange Road has probably, the most realistic portrayal of teenagers acting their age emotionally. Just like Teasing Master Takagi-san, however, it feels episodic.
Then, I think Your Lie in April is a very deep romance, but it is also super tragic.
Once again, I don't know what is your criteria for "good character designs". If you mean well-written, then I think those are very interesting and well-written characters. If you mean "good character designs" as in the visual art design, that is way too up to taste to know whether you'll like any of the ones I recommended.
